1099 Forms

1099 Forms for Laser Printers
  • There are three forms to each laser 1099 page (except 1099-R and 1099 MISC.)
  • All 1099's are sold in packages of 100 forms.
  • If you require a 4-part (except 1099-R), you would order 100 Copy A, 100 Copy B, and 200 State/Copy C. If you need a payer and state copy, order twice the quantity of Copy C as you did Copy A.

  • 5140: 1099R Federal IRS Copy A - Contains filing information for two separate recipients.
  • 5141: 1099R Recipient's Copy B - Contains two separate forms for two recipients to be filed with federal return.
  • 5142: 1099R Recipient's Copy C and/or State, City or Local Form - Contains two separate recipient forms.
  • 5143: 1099R Payer's Copy D and/or State, City or Local Form - Contains two separate payer reporting forms.
  • 5144: 1099R Blank - Features one vertical perf and one center cross perf.
